Authorization to Hold Credit Balances

I, ____________________ hereby authorize Fountain of Youth Academy of Cosmetology to:
Credit my account with the financial aid funds designated for tuition, books, kit, supplies and fees. This may include Pell Grant, Federal Direct Subsidized and Federal Direct Unsubsidized Student Loans for the student and Federal Direct PLUS Loans with parent approval.

I understand that once the above charges have been paid in full for any current semester or payment period my account may have a credit balance. Therefore, I request that Fountain of Youth Academy of Cosmetology retain the Federal Direct Loan and or Pell Grant credit balance on my account to help cover remaining tuition charges and/or any additional individual authorized expenses for subsequent payment periods or semesters that I might incur prior to the end of my current "year" ("year" meaning the end of a loan period in the case of FSA loan funds and the end of the last semester in the award year for which they were awarded in the case of any other FSA program funds, i.e. Pell Grant). At the end of my current "year", any Title IV credit balance still on my account will be refunded regardless of this authorization.

I understand that in the event of my withdrawal I will be responsible for the full cost of my kit and books since once opened and used they are for the most part no longer returnable for sanitary reasons, therefore in the event of my withdrawal from school, I authorize any credit balance retained on my account at that time to be used to pay for all applicable institutional charges plus any remaining charges for my kit and books incurred as a result of my withdrawal.

The specific time frame that you wish the school to hold your Title IV credit balance(s) for is the length of your educational program. This is notwithstanding the above stated regulatory requirement. Your credit balance(s) will be disbursed according to this requirement which means that after you reach the end of an Academic Year, any credit balance will be disbursed to you.

I understand that this authorization is voluntary and it is my right to change my mind about the school assisting me with managing my educational funds. If I change my mind, I understand that I may rescind the above authorization in its entirety or for any individual item at any time there is a credit balance on my account and the school is required to provide my remaining educational funds to me within 14 calendar days of receiving my written request. I understand that when I do rescind this authorization in whole or in part that it is not retroactive.

This request must be signed and dated by the student or their parent in the case of Parent PLUS Loan credit balances.
